How do I file a claim?

Please inform our Customer Support and Claims Assistance Team of any claim event and prepare the following documents, along with the filled-out claim form:

Basic Documents for the owner (if claiming for the owner):  
  1. Affidavit of Accident or Police report  
  2. Death Certificate 
  3. Scanned Original medical receipts 
  4. Medical Certificate /Abstract (for permanent disablement/ dismemberment due to accident) 
  5. Additional documents for Beneficiary:  
    1. Marriage, birth certificate and other evidence establishing the relationship of the claimant and the victim.  
    2. Barangay Certificate in case Common Law partner is the beneficiary. 
    3. Certification from League Official or Organizer that Insured was injured.  
 
Basic Documents for the pet (if claiming for the pet):  
  1. Affidavit of incident or Barangay Report or Police Report  
  2. Certification from the Veterinarian in case of death 
  3. Medical Certificate from the attending veterinarian 
  4. Original Medical Receipts 
  5. Barangay or Police Certificate of PET Non-Recovery 
  6. PCCI Certificate (to establish ownership between Pet and Owner)  
Please note that these are the basic requirements, and the insurer may ask for more documents if needed.
