OWN DAMAGE/THEFT OF VEHICLE PARTS  
  1. Police Report and/or Notarized Driver’s Affidavit/signed MARF (Motor Accident Report Form) 
  2. Photocopy of Certificate of Registration & latest Official Receipt
  3. Photocopy of Driver’s License with Official Receipt
  4. Photos of damaged portion showing vehicle’s plate number
  5. Repair estimate from Insured’s preferred shop 

CARNAPPED VEHICLE 
  1. Police Report 
  2. Photocopy of Certificate of Registration & latest Official Receipt. 
  3. Photocopy of Driver’s License with Official Receipt. 
  4. Original or Duplicate copy of Complaint and Alarm Sheet issued by Traffic Management Group (TMG) Camp Crame 
  5. Original copy of TMG Certificate of Non-Recovery (90 days after issuance of Complaint and Alarm Sheet)
  6. Secretary’s Certificate (if vehicle is registered under company) 

VOLUNTARY THIRD PARTY PROPERTY DAMAGE 
  1. Original copy of certificate of no claim issued by TP’s insurer 
  2. Photocopy of third party certificate of registration and official receipt
  3. Photocopy of third party driver’s license
  4. Photos of damaged portion showing vehicle’s plate number
  5. Preferred shop repair estimate 

VOLUNTARY THIRD PARTY BODILY INJURY 
  1. Original copy of Medical Certificate 
  2. Original copy of Hospital Statement of Account and official receipt/s 

DEATH CLAIM (CTPL/VTP-BI) 
  1. Original copy of official receipts of medicine bought outside the hospital 
  2. Original copy of Statement of Account and official receipt/s of Professional Fees
  3. Copy of Death or Post-Mortem Certificate
  4. Service contracts / invoice of funeral and burial expenses
  5. Copy of Birth / Marriage Certificate
